Helping Clients Navigate the Healthcare System
Since its founding in 1998, the Northeast Florida AIDS Network (NFAN) served as a trusted resource and support system for individuals living with HIV or AIDS throughout Northeast Florida. NFAN’s Medical Case Management program helps clients navigate the healthcare system and connects them with specialized providers who understand their unique needs.
Through this program, clients gain access to ongoing medical care, life-saving medications, dental treatment, and mental health services.
Connecting Individuals with Community Services
The role of NFAN’s case managers extends well beyond medical support. It includes providing co-pay assistance to reduce financial strain, coordinating reliable transportation so clients can attend medical appointments, and connecting individuals with additional community resources that promote wellness and security.
